Athletic WAIVER

ASSUMPTION OF RISK

I, the undersigned participant, acknowledge and fully understand that participation in athletic, recreational, or physical activities involves inherent risks. These risks may include, but are not limited to, slips, trips, falls, collisions, equipment-related accidents, dehydration, physical strain, bodily injury, serious injury, paralysis, or even death.

I understand that these risks may be caused by my own actions or inactions, the actions or inactions of others, the conditions of the premises at 1360 Okie St NE, Washington, DC 20002, the weather, equipment malfunction, or the negligence of Guerrilla FC, LLC, its owners, officers, members, employees, volunteers, contractors, and partners (“Released Parties”).

I voluntarily assume all such risks, known and unknown, including those arising from the negligence of the Released Parties.

RELEASE OF LIABILITY

In consideration for being permitted to participate in this athletic event hosted by Guerrilla FC, LLC on December 5th, I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me, or to my property, whether caused by the negligence of the Released Parties or otherwise, while participating in the event or while on or around the event premises.

INDEMNIFICATION

I agree to indemnify, defend, and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur arising from my participation in this event, including attorney’s fees.

MEDICAL CONSENT

I authorize Guerrilla FC, LLC and its representatives to take any action they deem necessary in the event of a medical emergency. This includes, but is not limited to, administering basic first aid, contacting emergency medical services, notifying my emergency contact(s), and authorizing medical professionals to provide examination, treatment, or hospitalization.

I understand that Guerrilla FC, LLC and its representatives have the right, at their sole discretion, to contact medical authorities or emergency responders if they believe such action is warranted, regardless of whether I am able to provide consent at that moment.

I assume full responsibility for any and all medical costs that may result from my participation or any emergency response initiated on my behalf. I certify that I am physically fit to participate and have no medical condition that would be aggravated by this activity.

PHOTO & VIDEO RELEASE

I grant Guerrilla FC, LLC full, unrestricted permission to record, photograph, or otherwise capture my likeness, image, voice, or performance during the event. I authorize Guerrilla FC, LLC to use, reproduce, publish, distribute, or display such media—now or in the future—in any promotional, marketing, archival, or educational materials, in any medium, without compensation to me. I waive any right to inspect or approve such materials.

SEVERABILITY

I agree that if any portion of this waiver is held invalid, the remainder shall continue in full legal force and effect.

ACKNOWLEDGMENT OF UNDERSTANDING

I have read this waiver, fully understand its terms, and sign it freely and voluntarily. I understand that by filling out this form below, I am giving up substantial legal rights, including the right to sue.
